Welcome to Children's Endocrinology & Diabetes Center

Your Pediatric Endocrinology Clinic in Savannah, Georgia. 

New Patient

As you are preparing for the appointment, you can do a few things to help expedite the appointment.

For this appointment, please bring with you:

  1. Your co-pay, co-insurance, and/or deductible amount (will be based upon your insurance plans contractual rates). Payment is due at the time of service.
  2. Your current medical insurance card and driver's license and information.
  3. Your child’s medical records sent from your Primary Care Physician/Provider’s office (if applicable).
  4. Copy of old and new blood test results (if applicable).
  5. CD copy of Old and/or New Bone Age X-ray images and reports (if applicable).
  6. We are a specialist office. If your insurance plan requires prior authorization to see a specialist, it is your responsibility as the guardian to obtain the appropriate authorization before your visit with us. Without this authorization, your insurance company will not pay for this visit and you will be responsible for full payment at the time of service.

In addition to the initial office visit, laboratory services may be performed. You may receive additional billing statements for these procedures directly from the laboratory.

NEW PATIENT INTAKE FORM & OFFICE POLICIES

  • Forms can be completed online through the PATIENT PORTAL or you can print them from our website and bring them with you to your appointment.


  1. General Consent (PDF)- All patients must provide their consent for treatment, communications (calls, emails, and text messaging).
  2. New Patient Demographic Information (PDF)
  3. Authorization for Release of Medical Information (PDF) - Allows patients to authorize the disclosure of their health information to a designated individual, company, agency, or facility.
  4. HIPAA Notice of Privacy Notice (PDF)- Describes how health information about you (as a patient of these responsibilities.
  5. Financial Policy (PDF) - This form advises patients of their complete financial responsibility for all medical services received without regard to insurance eligibility or coverage determinations.

We understand occasional circumstances prevent you from keeping your scheduled appointment. If possible, please notify our office in advance if you are running late or unable to make your appointment. Our scheduling coordinators will do their best to re-schedule you promptly.


Please arrive 30 minutes before your appointment time with your new patient paperwork completed.
